Ok so i need 2 peices of ID, and i need a model release form?
my 2 questions are where can i get one of those? or do i write it from scratch? and if thats the case is there a template i can maybe see?

and is there anything else i need to make this official?

talk to a lawyer. he can not only give you necessary forms but let you know if what you're doing is legal. it has to be legal to shoot the porn in your state, county, and community. breaking these laws could make you a sex offender, i'm sure you dont want to be a publicly listed sex offender for the rest of your life.

Two forms of ID is good, only one is needed.

19 in canada? wow i thought it would be younger then the states if anything

In this business it's always good to run things by an attorney especially if you are going to be producing content. Laws and regulations are different in each country, so you want to make sure you have everything in order.
