"Tranny" comes from "transvestite," and the "vest" part of that word refers to "vestments" or in more modern terminology, "clothing." A transverstite wears the clothes of the other sex.
A she-male has a mix of the physical attributes of both sexes, most commonly breasts AND a penis (though the breasts are frequently not real and the penis not able to get very stiff).

A transvestite is a person who wears clothes and/or assumes the appearance of the opposite sex.
A transexual(Tranny) is a person who has gone through a sex-change operation. A pre-op transexual is one who is taking hormone/testoterone shots or has had a part of the sex-change procedure but not all of it. Shemale is usually slang for pre-op male-to-female transexuals.
A hermaphrodite is a person born with both male and female sexual organs.
