I'm not seeing that site in the SERPs but I pulled up the URL. That is damn odd. The only
thing I can think of is either a) the site once had MUCH different content than it does now and Google just hasn't caught up yet or b) there are a shit-ton of 'buy viagra' backlinks pointing to this site - or maybe a bit of both.

the site is old and has never had spamy viagra content according to the wayback machine!

It was probably serp hacked. They install a script that detects the user agent, and when it is a se bot they show different content. So a normal user of the website sees the normal content, but the bots see spammy keyword rich text with affiliate links.
