Not true at all, there are dominant males who force their wives to sleep with other men. The more common would be the female dominant cuck, but I assure you there are both.

Good question.

When I hear "swinging", I think two couples.
When I hear "cuckold", I think one couple and one other male.

If the cuckold male "swings", then is his attention on his mate or the other female?
Doesn't seem like this would work for the cuckold male because his focus is on
what his wife is doing thus leaving the other female with nothing to do.

It's still an area that I don't understand, I'm just going on what I see on the net.
