Ok the main issue is that due to atrophy of the muscles your circulation sucks ass. See over time your leg muscles shrink a great deal since they have no real use any more. By the way this is sort of a good thing cause lifting dead weight with your arms is a huge pain in the ass, so the lighter your legs the better. Back to the point. When your leg muscles have shrunk a great deal they no longer circulate the blood as well. Your legs do happen to have a crap load of muscle tissue in them as you may have noticed in comparison to the rest of your body. So when you stand your heart literally has to work extra hard to pump the blood properly.
Then you must combine that with the tendency for your muscles to store up or pool the blood (swelling). Since they do not constrict much and push the blood back up your body it makes you very woozy to say the least. Oh and a small pin prick in a swollen limb like a foot will bleed somewhat akin to a Japanese anime character in a fight scene.
Lastly is the spasms issue. I have fairly bad spasms and well they do not go along well with standing up.
