I suggest you contact Mitch at Netbilling. He keeps the fraud out himself. Yes, he sets you up with a merchant band then he looks at every charge and makes sure they are kosher. I don't know how he does it but he does it well. I think he has help from little merchant elves though.I've trusted my sites to them for over a decade...

I do. Setup a support ticket system, and process refunds when requested.
As for fraud it's not hard there are a lot of fraud checks built into the gateways plus some external stuff.
Not hard at all. I use card services.
