It can mean a lot of things but it usually means that they are done with you and ready for someone else.

Some possible meanings:

I've cheated on you.

I no longer find you attracted.

I have an opportunity to date someone a whole lot better than you.

I am shallow and have no idea what I'm talking about, but will regret this decision in the future when the person I'm leaving you for, leaves me.

You've gained a lot of weight and disgust me, a hotter person said they like me.

Well it also depends on the situation at hand. Let's say the girl desperately wants kids, and you absolutely don't want any. He wanted her back, so they flew out to Vegas and got hitched a couple months ago.
