The "failure" example is actually one which shows how it should work: if a large number of relevant sites associate a specific term with a specific site, that site gets ranked highest. The "failure" spam wasn't spam, exactly because it was being done by a large, diverse group of non-spam sites, something almost impossible to entirely emulate for commercial sites.
