The album cover is symbolic for the end of the relationship between Pink Floyd and Syd Barret. Syd Barret, the founder of Pink Floyd left the band after he went insane. The guy is on fire, symbolizing Pink Floyd getting hurt by Syd, and the handshake symbolizes the relationship between the band and him. The whole album, Wish You Were Here is the band mourning the loss of their friend and founder. It works on two levels, they wish he was there in terms of his senses, and also physically with the band.

maybe you know the story about this song.Originaly it wasn't palnned to be recorded.During recording in studio there was a guy with tooth-brush all day watching them working.They didn't recognize Syd as he was fucking fat.Some1 told them - jesus its Syd Barret - then Roger wrote this song and added to album.
