Sometimes insurance will pick it up. I was on one PS forum and a lady had her insurance pay for her rhinoplasty because her doctor diagnosed her as having a 'breathing problem caused by a nasal deformity' or something like that.
I paid for my surgery by working like mad for a few months and saving the money. Other people I've met online have gotten bank loans or even financed it through companies like Medicredit. On one forum, a woman went 28,000 in debt (via loans) to get everything she wanted done at once.
One thing I do advise is making sure that you have money for the surgery on hand, plus some extra money in case things don't turn out the way that you want. That way you can have it fixed right away instead of having to wait to get more money.
