Don't confuse model releases with 2257 info. A model release is to protect you from the model. If done correctly the model release, and the 2257 information she fills out are on different contracts, so you don't file them together.
But to answer your question... I'm no lawyer, but my lawyer just told me a photo almost like that was not in any way shape or form covered by 2257. If you're producing it, like always get it all anyway, but you don't need it.
