Maybe profound thought and/or relfecting? Like when people are nervous they tend to avoid eye contact and in more extremem cases become shifty(hands, body weight). He may have been considering a treatment while bearing in mind what meds cause problems if they're mixed. or maybe he was talking about one thing but thinking how much he should fleece her at the same time?
