These 'fetish' guys will pay much more willingly for something, which turns them on, disregarding site design, navigation, higher price etc.
Choose something that you understand or at least talk to someone who understands particular fetish and ask for advice. Then you'll understand the difference and be able to target your ads correctly and communicate the right thing.
