I used to do special effects for mainstream movies... I can tell you that from what I know, I'd say it's real.
The thrower definitely has the strength to get the scissors into the kids arm.
The catcher is most likely in shock, and doesn't feel the pain just yet.
The buckling of the skin around the entry wound looks real. Your arm is very meaty right around there and I'd think you have less nerve endings on your arm in that area.
I can't tell you how many times I've seen two guys punching each other in the arm in that area. It's just a fleshy, meaty, fairly padded, not too receptive to pain, part of your arm.
Also, if the scissors were real ones that they cut to fit his amr and hid it with makeup they'd be flopping around a bit. In the video those scissors appear to be firmly planted in his arm. Anything glue to the surface, no matter how light it was, would still flop around. Your arm is very fatty right there it's hard to glue shit to your arm and not have it flop around.
