I do a little bit.. for some out there micro niche keywords.. I generally make about 2x-3x what I spend, but the total amount is smallish since there isn't a lot of traffic on keywords like midget amputees. ;)

I do not have any porn sites. I should have specified mainstream.

It's definitely a front loaded expense. You burn some cash and time tweaking your keywords. But it evens out in the long term.

I use them for a non adult academic site.

Google keywords is for very well targetd keywords like

Oklahoma State Flag

or such.

Its a waste to use keywords on google for adult, most people know its just more spam pages.
