Making your web images EXTRA CLEAN AND CRISP - Any Suggestions?

I generally design it 3 times the finished size but for some reason, it's not working too well for me any more. Not sure why, it just isn't.
Any suggestions?

How much more explaining can I possibly do? I'm just asking for suggestions on how to make web images (buttons, banners, etc) really crisp and clean.

Photoshop can only clean up an crappy image so much.. take better pics

I always use as little jpeg compression as possible in the 'save for web' dialog and also add some unsharp mask.
