If you arent ripping out you should probably keep your RPMs lower when starting in 1st gear. On my bmw I keep just above 1K when starting in first.

I've driven both ways, my cousin has had a Skyline here in the US since 96 and I have driven it enough to be very comfortable with shifting with the left. I personally prefer driving on the left side of the car and shifting with my dominant hand, it feels more natural to me. I always find it odd to see someone changing gears with their right hand (i.e. in the US). For some reason I just can't imagine doing it - isn't it harder to control the wheel (e.g. palm it) with your left hand?
Meanwhile, nothing wrong with revving. I normally stay in second gear up until 70kmph in city driving, and that's in a saab 9-3.

In the US we are odd about high RPM's. It's cool sometimes and other times it's immature. The difference between a saab and a whored neon is that at 5k the saab won't sound like a lawnmower.
