I was totally in this exact same situation about 8 months ago. Difference is I had a girlfriend at the time (was actually engaged). My ex-gf, who wouldnt ever put out when I dated her, all of a sudden looked at me one day and said "Fuck me, Im ready". I passed on the opportunity, and her next boyfriend took the V-card. I regret it all the time.
Fuck her, and worry about the rest later. She knows what the deal is, just do it.
