I can't remember who it was, but I helped them go from 3 to 4, well actually, tmm moved them from to 4, did everything if I remember correctly. Our end required a join page upgrade, but the old ones work, some tour stuff changed out... something with the aff content, was a pain.. and then some simple skin changes, a new content type added.. all simple shit. And I think we had some ip cleanup's with processors.
I don't recall ever having any real issues. Only half problem is the lack of admin stats.

I tried to make the upgrade but it was really painful. Too many problems and not enough help from TMM. I am still running the previous version and I think is where I am gonna stay.Comment

We are using it on http://LiveCamCash.com and the older version at http://BigBucksNow.com ... I'm loving 4.0! We plan on upgrading BBN to it as well.Comment

just be careful if your affiliate side is skinned - that will require completely redoing.
Apart from that, any custom smarty functions will be preserved, TMM will take care of the nats3->4 move for nats core and I think billers postback urls change for some billers, but nothing amazingly out of the ordinary for you.
Some things are a lot lot smoother in nats4, others are less so. However, whenever development halts on one version, it's time to move over to the new version or get left behind.
Migrating from 3 to 4 will only get more difficult the longer you leave it....
