Fucking Madagascar... well I've beaten it ... it's kinda lame but it works..

1) sell all your symptoms... (makes it harder for you to be caught onto)
2) buy all level 1 resistances except for level 2 on moisture and all the way on drug resistance.
3) Wait till you get 30 points or so then buy all the Transmissions
4) Watch it spread like wildfire... If Madagascar closes it's borders just restart... you've lost... If you start out on Madagascar you've struck gold....
5) start buying symptoms (go straight to level 4 symptoms and get the ones that kill people)
6) WIN!

did you start with Realistic or Relaxed? and what disease did you pick to start?

Doesn't matter it works both ways.... Realistic just moves faster.. I always play on the fastest speed too... Shit takes forever if you play it any slower...

If you start in Madagascar make sure you sell your symptoms so they don't catch it and close the ports on you (keeping the virus in Madagascar)
