Make sure to show this to your current accountant and get a clear answer from him/her if the refund was due or not. In my teens when I filed my own paperwork with the IRS, I received two refunds unexpectedly. Shortly thereafter the IRS requested the monies to be returned them, unless I disagreed. Since the money was spent, and it was the IRS who figured I was owed a refund in the first place, I did disagree and ended up going through a nasty audit.

Refusing to return the $3000 refund covering two years, costed me well over $20000 with what they found in the audit (penalties included).

If it turns out the account made the mistake, and didnt realize you were owed money, fire him or her, and take your previous years return to another accountant immediately for a review. If money is owed you for previous years, the accountant may refile your taxes for those past years.
