Obviously, there are plusses to being Hugh Hefner, but I think he earns them.
I imagine that, Viagra and Cialis notwithstanding, Hugh Hefner is constantly reminded of his own mortality, being the age he is and surrounded by that much young ready-to-go flesh.
Having so many people around also means there are always witnesses if he has a bad day.
