Looks like you had a lot going on in 2003 Gerco. Was it just the newness of the site or did you do a lot of promoting back then?
Extreme Insertions is a very distinct niche and I would think that there are certain countries, i.e. Germany, that might enjoy your content more than other countries. Maybe consider translating your site into German.
I would try to identify other blogs or sites that cater to a similar type of audience and see what you can do to work with them.
Just trying to make a few suggestions.

Dang, I was just thinking of you today, so I think I'll revive this thread. How do people know that you have new stuff. You've said that there's a community out there that bounce around. Are you keeping them sufficiently excited. Your tour looks the same no matter what new stuff you might have inside.
Have you checked tubes? extreme insertion videos get a lot of views. Probably mostly curiosity and freak factor, but some cut up preview videos on the tube sites might drum up traffic. Give them a taste of the stuff that your people can take and then bam, end the video right at the insertion scene. If you need fresh new stuff, do you already have The The Cannon
