I'm in a similar boat as you, but here is what I discovered after a number of years trying to find what my "problem" was.
First, get checked for sleep apnea. You don't have to be overweight to have it, and some don't even know they do. Not getting a full nights sleep will have adverse effects on T production.
Secondly, if you are overweight, the more body fat you have, the more estrogen you will produce. So overweight men tend to have lower T numbers that those of their fit counterparts. A friend of mine is in the mid 300 lbs and his numbers where in the low 100's. after a huge lifestyle change and medication, he's now around 230lbs and T of mid 600 withOUT meds.
If you messed with steroids and did not cycle off properly, that will certainly have an effect on your gonads. I've had my far share and admit, didn't use things properly and now pay for it later in life with thyroid and gland problems.
Get a few things checked first, there is always a way to help, however might require meds for life depending on the damage.
To answer your question, YES you should start therapy if your levels are low or low enough to warrant medication, otherwise you could be doing even more damage.
