So it looks like you didn't give him any chance at all to opt out for the charge May 7,2013 (there was no way to opt out per the screenshot above - it was not just a matter of opting out too late) however you would have gladly opted him out for next year. That is the main reason why he is upset. It also looks like you failed to communicate to him that he could not opt out when he initially said he wanted to opt out.
In addition to that less than a week after taking the $500 fee which he could not opt out of you have now unilaterally closed his account because he said some things on the forum about you. So he paid $500 for one week.
