Not keeping it clean, using poor quality magazines, or not loading the magazine properly are the main reasons.
They are really good rifles, but you have to take care of them. Take care of them, they will take care of you.
Any of the newer rifles with the M4 loading ramps machined into the chamber will be pretty much jam-free when taken care of properly.
