I'm reading this thread after it's all over and done with. Nevertheless, I'd like to point out for the record that Domain Holdings is a reputable domain brokerage firm. I've never personally purchased from them, but I have interacted with Peter regarding domains of mine for sale. If you read DNJournal's weekly domain sales reports, you'll find that Domain Holdings scores some very big sales on a regular basis.
The extra form was probably some standardized purchase agreement that they use. Just my guess ... For a $1,800 sale, it probably seems like overkill. Yes, it's a pity i waited a month, restart the transfer 3 times, wrote the same thing twenty times, before they forwarded to me the authorization email that name.com sent to the email address of the whois....i do a lot of domain transfers, and i never have all these troubles for transfer a name, and they should know very well how work these things...this is my story
