I've never personally purchased from them, but I have interacted with Peter regarding domains of mine for sale. If you read DNJournal's weekly domain sales reports, you'll find that Domain Holdings scores some very big sales on a regular basis.

The extra form was probably some standardized purchase agreement that they use. Just my guess ... For a $1,800 sale, it probably seems like overkill. But if you were purchasing a domain for $180,000, you might appreciate the extra form.
