I've noticed an obscure error with it as well over the last six months. Try clearing your cookies, exiting firefox, and then going to the form again. I bet it will suddenly work. I suspect it has to do with going to a form and then clicking back out of it. Something is screwing up the cookies causing the error. I never looked into it much because it's such an obscure error.
