At the end of this message is some legal jargon about what is obscene. But basically you are talking about two different categories of legality. There is what constitutes the obscene and what constitutes rape.

Generally speaking the obscenity laws have varied but what's been held pretty firm is this: Poop and piss appeal to a prurient interest (that is, the average reasonable person thinks poop and piss are disgusting; those who are sexually excited by poop and piss are deviants in the eyes of American law) and prosecuters will nail you for showing poop and piss in a sexual context. They might not get you right away but they can get you at anytime and it will stick. In other words, if you show models using the bathroom, have them sitting on a toilet.

If you live in Germany and host in the US, the government of either country could go after you. But they probably wouldn't know if, say for example, you get a web host and private domain registrar in a country like the Netherlands, possibly Eastern Europe, where the government usually doesn't give a damn.

I don't know where you live so laws are hard to quote or even to guess at. I will tell you that in the US.
If you go by the rule safe, sane and consensual. You will not go too far wrong.
Obviously kids beasts etc are out.
Torture is a word many of your link trades will object to (torment works much better). Any hint of "forced" i.e. non consensual is usually best avoided.
