I hear ya. Trying to get a winning paysite design today is a very long and/or spendy process. I am glad I am not in that game.
As far as sales, even if it's now free for all, it is still more convenient for those with money to buy a membership. Plus they feel they are not going to get hassled as they bought the item.. porn. So people still are buying.
BTW congratz on your new foot site. Will get it up and send you my few hits when I can.

Let's take these statements as givens:

How do we work WITH the ideas that there is lots of free porn out there and that the free porn is not going to go away?

Why would you want to work with free porn? It's a business. Samples are fine but free is never free. How is it done? You research and learn odd things people are into. Lifestyles, fetishisms, and preferences. Then you take it a step further. To quote a movie 'build it and they will come'. People get off on anything and everything. And those people have money to spend.
