If you have the time to manage it, you're much further off creating your own gateway and promoting your own incentivized CPA offers through it. They just offer a nice managed set it and forget it solution.
I think AAN and adult gateway programs have the potential to change the adult affiliate landscape massively. Maybe even in a good way if they are operated properly. The scary part is that CPALead appears to be a very strongarm sort of program that doesn't appear to care one way or the other about piracy. It's a silent giant waiting to rise. It'll be interesting to see how it plays out and if it survives..
