There's a joke about it at the end of one episode, a guy asks something along the lines "what's the deal with Stewie, does anyone understand what he says?" but the credits roll before the question is answered...
It's pretty clear that Lois can't hear him since most of her responses are generic. Brian can obviously communicate with him although he never seems to offer to interpret to others.

Many people say only Brian (the dog) can hear Stewie, but I've seen a few episodes where Lois answers to something he says.Comment

Not that I watch the show a lot, but there were times where Brian and Stewie did things to amuse/barter with others and they were understood by adults.
It's just another halfassed/halfbaked MacFarlane attempt at humor.Comment

Brian can understand him and people that don't know Stewie very well can understand him as well.
