That's a headset. An exaggerated one obviously to let potential members know that she does webcams. CM uses a headset when she does hers too for crystal clear audio so the guys can hear every moan perfectly, but of course there is no need for the headphones on the ears in real life for that.
From a marketing standpoint, I'd say that using the headset with the big headphones for that picture conveys to the surfer that she is interactive with them. That's what went through my mind the second I saw it. I think it's great marketing.
