Its not constantly hard, its not an implant or pump..they snip the tissue at the base to allow it to "naturally" extend the length about 1-1.5 inches...then to make it thicker they graft cadaver tissue around the circumference...
I saw a guy who had it done on one of those plastic surgery shows...several months post op he seemed very pleased and said it worked normally.
