Little bit more complicated than that, need some check boxes as well.. one sec, i'll find another example.

Something like that, we don't need as many boxes though... Maybe 12 check boxes and 5 text fields or so (name, phone, address, description of service).
I usually do my own but always seem to mess them up after the first few fields... lol
