The MSO's dont pay a lot these days because they know they can get away with it thanks to a glut of content. The companies who buy your content and then resell to MSO's have to do a lot of work like

1. Make 3 versions , X,XX,XXX
2. Build metadata in CabelLabs specs
3. Verify that all 2257 info are correct, they are not fake etc etc
4. Provide legal liability etc etc

And after doing all this they pay 7 - 10% of the revenue. I estimate last year around a billion dollars worth of porn got sold by MSO's plus hotels.
