You hit the key points:
1) If mankind doesn't destroy itself or the Earth
2) If we discover civilizations from other worlds or (more likely) they discover us
3) If they have a "Federation" and we are deemed worthy to join
Then we will primarily identify ourselves with our planet and secondarily
with our nationality much like many people identify themselves first with
their country and then their state or region, ie American from Texas,
California or Texas
It will only happen when it is more important to be human rather than Canadian, German or Australian.
