I didn't take a look at all, but Argentina, Andorra, Iceland and Italy to name a few have massive CC penetration. Now, whether to sell to them by phone or CC is another question. See, the main problem isn't CC disposal or lack of it, it's price and non-localized content, not even in the join page. But since nobody doesn't care much about the main page of their sites, well... it's quite easy to see one of the reasons.

Now, if you serve a page in the correct language where everything is clear and people isn't afraid to pull out their CCs because they don't know what are they commiting to, then I'd say CC is the right option.

No matter the common belief here, adult internet isn't (or shouldn't be) too far from mainstream internet. In general terms, what applies to mainstream (including ways of payment) applies to adult, hence, if CC card is the most common way to pay via the internet in mainstream (and in most countries) then it's also that way for adult. Now, adult pages trying to trick surfers (aka customers) with hidden cross sells or weird terms is the main reason to doubt before signing, esp for those that can't read the terms.
