I don't want to speak for them, but I'm 99.9999% sure that unless you are a US affiliate, you don't have to fill it out.

It helps prove that they are sending money to you (someone out of the country), it has 0 impact on you and is not even reported in Canada. So fill it out and send it back to them.

The form is intended for individuals with United States citizenship. So you don't need to.

Yes you do. US Sponsors should be doing this but most don't. And its applicable to all foreign webmasters, not just Canada. Its just a declaration of foreign income to the IRS, nothing more.

Nope. we aren't governed byt he IRS.
