its a fully developed piece of a fir tree. obviously it didn't grow in his lungs. most likely it was part of a branch or piece that was shoved down his throat, and part of it went down his windpipe. if you put it down someones throat pointing the wrong way... it isn't coming out easily... because the needles are directional... that would cause it to keep working its way downward into a lung
