I can only speak on the U.S. and coming from my years of telcom before adult.
Actually they are called network nodes, and they are located at key spots around the country. If you look at a map, they are typically where the 7 Baby Bells, main offices are plus a few others.
Knocking out any one of those will not take the whole internet offline, but you will cripple a lot of it by making things super slow. Which leads to some sites not being accessible, e-mail bouncing, hanging, and serious lag.
It's happened a few times around where I live when the Ameritech/SBC/AT&T node had trouble in the Chicago area a few years back. Luckily it's only happened a handful of times (around my area) that I can recall from memory.
