thats ok buddy, i will jsut look at one of the naughty america dvds and see if i can just transcribe what i read on their 18 and over screen.

I belive the second screen has to include the name and address of the custodian of records. (don't quote me)

Here are the ones we use.

The 2257 regs require that the 2257 screen go within one minute of the beginning of the video OR if you use closing credits, it can go at the end of the closing credits.

The disclaimer isn't legally required (at least federally), it's just an additional layer of protection to let people know they are about to watch porn.

Here are the two that we use on our DVDs. (*Read* the disclaimer...) If anyone wants the .psd files for their own use, email me (chip at gaybucks dot com)

You should really have a lawyer write up a custom one, Leaving something out or copying the wrong one could be very costly.
