I would have to say OC3 Networks and here's why..
1) Great pricing. Their dedicated servers with 5 megs of bandwidth is $99.95 a month. So at $20 a meg, I basically get a server for free.
2) Great support. I dont care how low a hosts prices are, if they dont offer 24x7 support, i wont do business with them. Also, just having 24x7 support is not enough. i need quick response times. OC3 responds quickly to support requests.
3) The gravy. To me the gravy is how willing a host is to go above and beyond what is resonably expected with a hosting provider. By this i mean how much will a host go out of there way to help you with a non-typical support issue (ie, OS, Networking, Reboot, etc). OC3 has come to the rescue when i needed some extra guidence with non supported applications.
4) Access to the people in charge. By this i mean, if i need something special or service is not to my expectations, can i speak directly to the owner. In OC3's case, the answer is yes. Ilan has been available whenever i have needed him and is always willing to answer questions whether hosting related or not.
I am a current customer of OC3 networks and have been growing steadily with them and will continue to grow with them. I have also been moving servers from other providers because OC3 has provided me with the above that other providers have not...

We host with National Net and they are by far the best host out there... if you are willing to pay the extra $$$.
I have also heard nothing but good things about Mojo Hosting and Techie Media.

I have been with Techie Media for years. I think they are probably the largest adult hosting company if not close to it.
They host big names like thehun.net and Ebaumsworld.com
There servers are all top of the line
24/7 tech support that is unmatched

Realitychecknetwork - Have had a virtual account with them for years and they've always kept me happy, virtually no downtime whatsoever.
OC3Networks - We have several dedicated servers with OC3 and their pricing cannot be beat. On top of that, I've never NOT been able to get ahold of tech support, and they have gone above and beyond the call of duty several times in helping us.

wonder how ph call nver got to me, but I had been in toronto, emails all come right to my cell phone and have never seen anything go more than a few hours from sales not getting answered on a weekend. Three? You only need two - Webair and NatNet.
We have been in the business since the late 90s and we have everything from a truck load of boxes to complicated load balance set ups and these two companies allow us to sleep at night without any worries.

I've dealt with all of the hosting companies in the industry.
All I can say is that it depends entirely on what you need your hosting for. With dedicated hosting, remember that hosting companies only differ on 3 things: hardware, support, and bandwidth. However, I've canceled accounts becaues of poor billing / accounting departments.
My top 3 change from month to month.
