only when all means of improvement and sporadic escape have been exhausted! seriously- when there is absolutely no hope (i.e. late stage terminal illness), the person is a drain (hemorrhaging $$$$$ or imposing excessive stress) on their loved ones, there is no more effective pharmacological relief for the person or coping family members, and all other alternatives have been exhausted! it does not hurt the person who exits, but has a horrific effect on the loved ones left behind.
