Maybe it's just the way she's standing?
I'm trying to picture it fast-forwarded 5-20 years... she's 9 months pregnant and bloated, in the stirrups, and her doctor is telling her to push while having an upside down and contorted "Coal City" staring him in the face.
That might be her kid's first image in the world.
Of course, there's always hoping she doesn't procreate.
