Fetishes aren't real well understood. Sometimes psychoanalysis can drill down to childhood memories where the fetish got imprinted -- think of a boy who got a look up some hot girl's skirt and spent his sexually formative years jerking off to the memory -- but very often, there's no simple explanation like that.

I know a guy who is in to watersports porn (piss and pee) and also likes medical fetish stuff. As it happens, he got shot in the guts as a young teen and spent many months in a hospital, with a hole in his bladder and the nurses always messing around with his catheters and bedpans and private parts. Are these two facts connected? Signs point to yes, but it would be tough to prove.
