I've been using
Streamate (streamate.com uses Mtree for processing) for about 6 months now. Great conversions, they pay out $35 on a free signup (credit card verification) and pay weekly with wires/cheque/epass.
Support is infallible, Liz responds faster than Im sure is humanly possible.
