Those ringtones are most likely not paid ringtones but from free ringtone offers which convert extremely well. Example the user has to enter his number to recieve what he thinks is a free ringtone, obviously if they read the small print they would see they are billed something like $6/week to recieve more but they are advertised as free which is why they convert.
