There are 2 ways (maybe more) to get a boob job, underneath the chest muscle or on top of the muscle.

If they are place on top instead of underneath there really isn't anything to hold them up and they do sag. Some woman *cough* think they look more natural that way because they don't end up under your chin. Of course, those woman probably didn't have them that big.
